RELEASE OF LIABILITY: In consideration of services, I, for myself, for my child, do hereby release Willan Academy of Music, its directors, officers, agents, employees, and the individual members of the consultation team from all liability with respect to my child and I waive any claim for damages arising from any cause whatsoever.

PHOTO/RECORDING CONSENT
By enrolling in Willan Academy of Music, the student or parent/guardian (for students under 18) authorizes the making and use of any photos, videos or sound recordings during his/her participation in Willan Academy of Music concerts and events for any purpose Willan Academy of Music may make or authorize without compensation to the student or parent/guardian.
